In Aotearoa New Zealand, several communities—particularly those in rural areas and with high Māori populations—have embraced more integrated models of care tailored to meet local needs. What began as a response to limited resources has evolved into a preferred approach, driven by the effective engagement of nurses working at the top of their scope of practice.
Despite the clear benefits and proven potential of these models, Aotearoa continues to lag behind international counterparts, particularly Australia, in the development and utilisation of a locally based Nurse Practitioner (NP) workforce. This represents a significant missed opportunity to strengthen primary care and improve health equity across the country.
Why is this and what can be done to change this status quo?
In the innovation sector, the concept of the "commercialisation chasm" describes the gap between a great product or service and its successful adoption by the market—often caused by a misalignment in understanding each other’s value proposition. A similar gap exists in the health sector, particularly in underserved rural and urban communities where access to care remains a persistent challenge.
While the lack of General Practitioners (GPs) is frequently cited as the primary issue, many of these communities already have highly skilled health professionals—such as Nurse Practitioners (NPs) or those on the NP pathway—who remain underutilised due to outdated models of care and limited professional recognition.
Too often, short-term solutions like international GP recruitment are used to fill immediate gaps, only for those practitioners to move on shortly after. What’s needed instead is a long-term strategy focused on developing and retaining local talent—particularly NPs who are deeply embedded in and committed to their communities.
By investing in the professional growth of local health providers and enabling them to practise at the top of their scope, we can create sustainable, community-led healthcare solutions that deliver real, lasting impact.
As a Nurse Practitioner (NP)-owned and operated practice, we are deeply committed to improving health outcomes across Aotearoa—particularly in rural and underserved communities. We believe that NPs have a critical and growing role to play in delivering high-quality, accessible, and culturally responsive care. This commitment is reflected in our active support for the next generation of practitioners, including hosting 12 NP graduates in 2025 as part of our clinical mentorship and development programme.
As the founders of a greenfield primary care practice, we have experienced firsthand what it means to lead transformation from the ground up. What began in a rundown facility has been rebuilt into a thriving, financially sustainable, and clinically impactful general practice—serving a highly diverse population representing over 54 nationalities and spanning a wide range of social and economic backgrounds.
Our success has been driven by embedding robust systems, processes, and a values-based culture drawn from both within and beyond traditional healthcare settings. This integrated approach has proven that with the right leadership, investment, and innovation, rural and community-based healthcare models can be both resilient and future-ready.
